Is the alt text not enough?

If not and you want something else to go with it, you will need to write or modify a plugin.

I have written a howto on creating a plugin. Just waiting for James (forum host) to add it to the wiki. (I did post it here, but the forum logic then banned me.)

Maybe I could use javascript to display the alt text just bellow the image.
